Failure of the soft palate to reach the posterior pharyngeal wall. It may be caused by cleft palate surgery, palatal or pharyngeal abnormalities or injury, or neuromuscular dysfunction of the velopharyngeal sphincter. It causes hypernasality of speech. (references)
